New patient forms help us get to know you, your health history, and your concerns before your first visit. Completing forms ahead of time allows us to spend more of your appointment focused on conversation, questions, and care rather than paperwork. If you’re unable to complete forms in advance, we’re happy to assist you when you arrive.

Integrative Dentistry is a fee-for-service practice, which allows Dr. Evans to recommend care based on your needs, not insurance limitations.

While we are not in-network with any dental insurances, we're happy to provide courtesy billing for patients with PPO plans to help maximize potential reimbursement.

Your initial visit includes a comprehensive exam and full series of digital X-rays so we can understand the complete picture of your oral health. This appointment is designed to be informative, unrushed, and centered around your questions.

Comprehensive exam

Your visit begins with a thorough head, neck, and tooth-by-tooth exam, along with a deep dive into your medical history and current concerns. Dr. Evans takes time to understand what’s going on in the context of your overall health, so nothing feels rushed or overlooked.

Digital X-rays

We take a full series of low-radiation digital X-rays to see what’s happening beneath the surface. These images allow us to evaluate your oral health safely and accurately, and Dr. Evans will review them with you directly, explaining what they mean in clear, approachable terms.

Treatment recommendations

At the end of your visit, you’ll sit down together to review findings, photos, and X-rays, and talk through what it all means for you. Rather than pressure or one-size-fits-all plans, recommendations are thoughtful, conservative, and centered around your questions, priorities, and long-term health.
